Contact details and medical emergency form

Your child's safety is our priority. This form is designed to collect important contact details and medical information so we have everything we need in the event of an emergency.

This form must be completed by the parent or guardian of each pupil ahead of their initial taster session and at other times when this link is provided.

Child's medical details

Does your child have any existing medical conditions including allergies?
Does your child take any regular medication or require the use of an EpiPen?

Medication

If, in exceptional cases, you request a medicine to be administered, this must be handed into the School Office. It must be clearly marked with the child’s name, dosage and time to be administered. You will be asked to sign a Medical Procedures form for each administration. All medicines must be collected by an adult at the end of each day. Medicines, both prescribed (antibiotics) and non-prescribed (Calpol/Nurofen) are given to pupils in school by the School Secretaries or the Head's PA. In the event of both the School Secretaries and the Head's PA being unavailable, the Headmistress and Deputy Head of Nursery may dispense medications. A digital record of any such dispensing will be sent to parents stating the dose and time any medication is given via ParentMail or by email.

I understand that those supervising my child are in loco parentis and must exercise a standard of care that would be expected of a reasonably prudent parent. My child is fit to participate in all aspects of the school curriculum.
